For Windows 95/98:

  Go to www.3dlabs.com

Get BOTH the latest and the next to latest reference drivers.

Install the next to latest drivers. Reboot. Install the latest
drivers.
Don't reboot when it prompts you.

Now make a temp directory and put the OPENGL.EXE or whatever it is
from the next to latest drivers in it. Run it. It will spit
out the OpenGL32.DLL, GLU32.DLL, and some .h (header) files which
  you can ignore.

Put the DLL files in your C:\WINDOWS\SYSTEM directory.

For Windows NT: (this is what I'v eheard, haven't used it with NT)

  Go to www.diamondmm.com

  Get the latest NT drivers.

  Install.

That should work fine - now you should be able to select "Default
OpenGL" from the Quake Video menu and have it work properly.

Installing both the next to latest and the latest reference drivers
is probably just superstition, and I probably did something ELSE that
made that seem to have an effect, but it still looked like that was
what worked. Just remember you'll need 3dlabs' OpenGL DLLs from the
next to latest whether you install the next to latest or not. They
don't come with the latest.

If you have a Cyrix, god help you.
