--Making the Texture--
Use whatever art prog you have.  Favorite among the common folk is paintshop
pro ('cause it is free) which you can get from http://www.jasc.com
In this case, however, you have mentioned using 3ds max ($$$$).  I am left to
deduce that if you A) bought it then you can get/afford photoshop/illustrator
and should use those or B) pirated it and can do the same.  Basically, if you
can get your hands on Adobe (or Fractal [painter etc]) products use them.  If
not then use PSP.  The creative part is up to you.
"Ok ok, I've got my rad software. Now what?"
More fun! Heh...Now you have to draw/make the art.  However, things to remember
include:
    a)Use 64x64 as standard size.  You _can_ vary this however
    b) most important!! You must use the quake (2?) palette.  1 and 2 have
different palettes btw.  I don't have links to those off hand (anybody
else...?).  If you do not use these colors quake will not be able to use your
tex.
    c) Save it as a PCX.

"Got my PCX...now what?"
Ok...now go to 5thD (http://www.portal.ru/quake2/index.html).  These guys make
a program that you are gonna need called PCX2WAL.  For the mentally impaired
out there, this converts your PCX file to WAL format (which can be used by
quake).  Read the readme for usage.

"Got my .WAL, next?"
Now comes the tough part (or part that I have little knowledge of).  Since you
are using Qoole (which I love btw...not slamming it) you cannot as of yet
import custom .WAL files.  What to do what to do.
    a) Beg our beloved yet overworked programmers Matt and Paul.  I think that
this must be one of their next priorities besides completing the entity
properties.
    b) Download *gasp* another editor.  I know Worldcraft w/Quake 2 support is
neither out nor will there be shareware so rule that out.  Perhaps try QEd or
ummm QE4 (honestly I can't remember the names of those inferior editors ;).  No
personal experience here.
    c) Find a wad editor and insert your WAL into your q2 wad.  Some people out
there have more custom tex experience than me (Opiate comes to mind) and
perhaps can help out here.

PS: I just searched around for the quake 2 pallete and didn't find
anything....I know I saw it somewhere though ;<
